Scientifically Supported Health Benefits of Noni
1. Potassium
Noni juice is a real source of potassium. The compositional dossier EFSA assessed for noni fruit puree records 214.3 ± 56.9 mg per 100 g [1], and an independent laboratory analysis of commercial noni juice measured 56.3 mEq per litre — about 220 mg per 100 ml — in the course of investigating the juice as an unrecognised potassium source for kidney patients [2]. Both figures sit above the level at which a beverage may be described as a source of potassium in the EU. Potassium contributes to the maintenance of normal blood pressure, to normal muscle function and to the normal functioning of the nervous system.
2. The iridoids, and why they matter for authenticity
Noni's characteristic compounds are iridoids rather than the polyphenols that dominate berry juices. Deacetylasperulosidic acid is the most abundant of them, with asperuloside, asperulosidic acid, scopoletin and scopolin alongside it; an analysis of commercial noni juices on sale in South Korea quantified all five in the same samples [3]. These compounds are specific enough to the genus Morinda, and stable enough in processing, that they are the markers laboratories use to confirm a noni product is what it says it is [3].
3. Unusually little sugar for a 100% fruit juice
Noni is among the least sugary fruits anyone presses for juice. EFSA's compositional data for noni fruit puree give 1.3 g of glucose and 1.1 g of fructose per 100 g [1]; an independent analysis of Cambodian noni juice found 11.9 g of glucose and 8.2 g of fructose per litre, 20.1 g per litre in total, or about 2 g per 100 ml [4]. Most fruit juices carry five or six times that. What noni has instead of sugar is acid — malic acid at 4.64 g per litre, and a pH of 3.7 [4] — which is a large part of why it tastes the way it does.
4. Markers of oxidative stress
In a double-blind, placebo-controlled trial in 285 heavy smokers, 30 days of noni juice at either 29.5 ml or 118 ml a day lowered plasma superoxide anion radicals by 26.9% and 30.8% respectively, and lipid hydroperoxides by 24.5% and 27.3%, with no significant change on placebo [5]. Two things belong beside that result. The trial was funded by a noni manufacturer, and the product tested was not pure noni juice: the preparation assessed in Europe under that name is a blend of 89% noni juice with 11% grape and blueberry juice concentrates [6].
5. Blood lipids and C-reactive protein, in smokers
In a randomised, double-blind, placebo-controlled trial in 132 heavy smokers over 30 days, the same juice was associated with lower total cholesterol, triglycerides, LDL cholesterol and homocysteine, higher HDL cholesterol, and a 15.2% fall in high-sensitivity C-reactive protein; the placebo group did not change significantly [7]. The companion finding is worth as much as the result itself: a review of the human noni studies, compiled by the manufacturer's own research group, records an open-label study in non-smoking adults over 40 with normal or borderline cholesterol in which noni juice made no significant difference to any lipid measure [8]. Whatever these trials are detecting appears to need an oxidative load to act on.
6. Tolerability at high intakes
Noni juice has been formally tested for tolerability rather than simply assumed to be safe. In a double-blind study, 96 healthy adults took 0, 30, 300 or 750 ml a day for 28 days; no dose-related adverse effects appeared, and the noni groups reported 20–50% fewer adverse events in total than placebo [9]. EFSA reviewed the published case reports of acute hepatitis that had been linked to noni and found no convincing evidence of a causal relationship, concluding that consumption at observed levels of intake is unlikely to induce adverse liver effects [6]; a later opinion judged noni fruit puree and concentrate safe for the general population while noting that a small number of individuals appear to be particularly sensitive [1]. In a survey of 1,145 European consumers, around 90% drank between 30 and 90 ml a day [6].
7. Traditional use, and an unusual regulatory history
Noni (Morinda citrifolia) is a member of the coffee family that Polynesian voyagers carried across the Pacific, and it became one of the most widely used medicinal plants in Pacific Island cultures. In the European Union it holds a status very few fruit juices do: it is a novel food, examined and authorised on its own merits by Commission Decision 2003/426/EC of 5 June 2003, which also requires the words “Noni juice” or “juice of Morinda citrifolia” to appear on the label [10]. Hardly any juice on a European shelf has been looked at in that much detail before being allowed onto it.
